This webinar, hosted by Hospice News, details Heart to Heart Hospice’s journey after deciding to adopt Person Centered Care Plans.

HCHB’s Person Centered Care Plan equips organizations with the tools needed to empower clinicians to create effective, regulatory-compliant care plans. Clinicians can tailor the problem statements, goals, and interventions based on each patient’s unique needs. Ensuring that goals are measurable and that interventions cover all necessary services is a critical aspect supported by the PCCP. Like any tool, these outcomes depend on strong vendor-client collaboration and effective implementation. 

When Heart to Heart Hospice learned about this new Homecare Homebase feature they saw an opportunity to bring Person Centered Care Plans in as part of their initiative to revamp documentation. After adopting PCCP, they saw improved compliance and positive feedback from surveyors without increasing documentation time for clinicians. Their internal compliance teams observed that clinicians included more appropriate problems, interventions, and goals. Heart to Heart’s change management strategy led  to minimal tickets for support throughout their implementation of PCCP. Clinicians can create compliant care plans that focus on the needs of patients and their families without spending more time documenting.

In August of 2024, a surveyor reviewing Heart to Heart’s care plans using PCCP stated that: “Heart to Heart Hospice has the best personalized care planning with the most thought-out goals ever seen!”
