This metric has become well known in recent years due to new value based care models and proves to be a measure important in patient care and outcomes.
  • The Hospice Quality Excellence Award recognizes Hospice agencies with a score of 100 in the Hospice Quality Reporting Program (HQRP).
  • The Hospice KPI Excellence Award recognizes Hospice agencies performing in the top of their size cohort for care provided at the end of life. This award is based on the The Hospice Visits in the Last Days of Life (HVLDL) metric which  concentrates on visits by at least one RN or Social Worker in the last three days of life.
