HCHB is proud to support our customers across the home-based care industry. HCHB customers are large and small and provide a wide variety of services, but they all have one thing in common: a dedication to quality care. Each year, Homecare Homebase recognizes customers that exemplify a commitment to the home-based care industry with the HCHB Client Success Awards.
2024 HCHB Client Success Winners
Congratulations to the 2024 winners of The HCHB Client Success Awards. The winning organizations were announced during the 2024 HCHB Users Conference in Las Vegas, Nevada.
HCHB Client Success Awards: Quality and KPI Excellence
- The 2024 Home Health Quality Excellence Award recognizes Home Health agencies achieving 5 stars in the CMS published Quality of Patient Care star rating.
- The Home Health KPI Excellence Award recognizes Home Health agencies performing in the top of their size cohort for Acute Care Hospitalizations. This metric has become well known in recent years due to new value based care models and proves to be a measure important in patient care and outcomes.
- The Hospice Quality Excellence Award recognizes Hospice agencies with a score of 100 in the Hospice Quality Reporting Program (HQRP).
- The Hospice KPI Excellence Award recognizes Hospice agencies performing in the top of their size cohort for care provided at the end of life. This award is based on the The Hospice Visits in the Last Days of Life (HVLDL) metric which concentrates on visits by at least one RN or Social Worker in the last three days of life. This metric measures care provided in the most vulnerable of times.

HCHB Client Success Awards: Industry Impact
- Innovation and Achievement Award is a nomination-based award that gives Homecare Homebase the opportunity to highlight organizations that have found creative and innovative ways to address challenges within the ever-changing home care market.
- The Community Impact Award is a nomination-based award that will be given to an organization whose mission and efforts are positively moving the needle and making a significant difference in their community. Through extraordinary actions or programs, lives within the community are improved.
- The Industry Advocate Award is a nomination-based award that gives Homecare Homebase the opportunity to highlight the contributions of specific individuals for their excellence in patient care, performance, innovation, or regulatory affairs.
INNOVATION AND ACHIEVEMENT AWARD WINNERS
HarmonyCares
HarmonyCares has a thirty-year legacy of providing high-quality, personalized healthcare in the home for people who have difficulty accessing care. In 2023, the company set goals to improve the efficacy and efficiency of their operations by:
- Improving productivity
- Reducing redundancy
- Improving quality and outcomes
- Providing clinical decision support across the continuum of care
Early statistics comparing HarmonyCares’ 1st Quarter 2023 to 1st Quarter of 2024 show:
- 10.2 % reduction in workflow tasks
- 60% improvement in total time to completing all visit notes
- 67% improvement in documentation done in the home (for active users of the dictation platform)
- 16.5% reduction of readmissions in the first 30 days
- 7.8% reduction of readmissions in the first 60 days
VNA of WNY
VNA of WNY became the First Home Care Organization EVER to be Independently ISO 9001 Certified. This certification included a survey with a review of the company’s Quality Management System, (QMS) by DNV Healthcare surveyors.
VNA of WNY’s processes from after-hours cover and emergency preparedness to product storage and equipment maintenance were all carefully reviewed. The surveyors also sought out information on the organization’s active process improvement plans.
COMMUNITY IMPACT AWARD WINNERS
St. Croix Hospice
St. Croix Hospice won the Community Impact Award for their multifaceted bereavement and volunteer programs. The hospice offers several complimentary services. These programs include:
- Virtual and in-person support groups
- Children’s Backpack Program, a bereavement initiative that provides families with age-appropriate grief resources for children and teenagers.
- Lighthouse End-of-Life Planning Program™ provides complimentary public resources to individuals aged 18 or older to help them take control of the future and empower their loved ones to make informed decisions based on their wishes.
The St. Croix Hospice Volunteer Services team also provides companionship and support for patients on-service. Tuck-in volunteers help patients and their families prepare for the weekend. The Voyage Vigil Program™ provides companion volunteers to sit vigil with patients nearing the end of life and Pet Therapy volunteers provide patients with companionship using certified therapy dogs.

Stillwater Hospice
Stillwater Hospice has also been selected for the Community Impact Award for their establishment of the Peggy F. Murphy Grief Center. Stillwater Hospice has been servicing hospice patients and their families in Indiana for many years. About 10 years ago they built the Peggy F. Murphy Grief Center campus. The organization offers grief counseling and groups to the community. Stillwater’s bereavement outreach has grown as the company has expanded into more areas of Indiana. The company uses HCHB’s Bereavement Console to coordinate their bereavement services and help them continue to grow and serve our communities.

iNDUSTRY ADVOCATE AWARD WINNERS
Rachel Witt, Aveanna Healthcare
Rachel Witt, RN, BSN, Chief Clinical Officer at Aveanna Healthcare uses her great knowledge and leadership skills to make a difference in the lives of Aveanna’s employees and patients. Rachel is an advocate for patients pushing for innovative clinical strategies to achieve high standards of practice. She also represents Aveanna in state and national associations and is a direct resource for Aveanna’s Government Relations team, actively advocating for patients and families to receive care in their homes.
Ashley Jones, BrightSpring Health Services
Ashley Jones, RN, CHPN, has served as the VP of Hospice Clinical Practice and Quality for BrightSpring Health Services since 2022. During her 17 years in the hospice industry, Ashley has shown steadfast leadership, ensuring BrightSpring agencies meet the highest standards of care by advocating for quality improvement initiatives, promoting best practices and addressing patient and clinician satisfaction. She has improved company education programs by creating Hospice University. This program provides an excellent overall learning experience and ensures comfort and confidence using HCHB and PointCare as well as other platforms used on a daily basis. She has also collaborated with BrightSprings’ IT team to develop resources for clinicians and created an RN preceptor program to nurture and develop nurses. She has also lent her expertise to the industry as a speaker at the 2023 National Hospice and Palliative Care Organizations (NHPCO) annual conference.
