Forcura and Homecare Homebase Elevate Partnership

Two industry leaders to deliver technology integration that streamlines care transitions and strengthens smart referral management

Dallas, TX and Jacksonville, FL, June 08, 2023 – Forcura, a healthcare workflow management company, and Homecare HomebaseSM (HCHB), the nation’s leading software for home-based care, today announced an expanded relationship expressly designed to solve some of the home-based care industry’s most pressing challenges. Through the evolved partnership, which began in 2018, Forcura and HCHB will help providers better accept patients into care, maximize efficiencies and strengthen their market positions.

The two companies are collaborating to ensure that Forcura’s newly enhanced referral management solution, available to the general market this July, is fully integrated with the patient chart in HCHB. Most notably, the solution will act as a referral hub, featuring functionality that enables clients to manage all referrals in one place regardless of referral source and will allow key patient referral data to automatically populate in both Forcura and HCHB systems.

This added functionality, powered by artificial intelligence and machine learning, will help to streamline care transitions for patients and provide clients increased visibility, faster processing and smarter insights on their business performance and referral partner value.

“From the start of our partnership in 2018, Forcura has shared a vision with HCHB on the role of technology and its ability to empower better patient care,” said Annie Erstling, Forcura’s chief operating officer. “As we continuously innovate the Forcura platform, our coordination with HCHB ensures that our mutual clients have the best possible experience with their technology – and have more time to focus on their patients.”

Since Forcura first became a partner of HCHB, the two companies have delivered significant impact to home-based care. Each month, Forcura’s platform, integrated with HCHB, helps more than half a million patients receive efficient, impactful care.

“Over the past five years, Forcura has served as a strong and collaborative partner that aligns with our goals of helping practices do business faster and smarter to ultimately enhance patient care,” said Scott Pattillo, CSO at HCHB“This expanded relationship will ensure HCHB clients have access to some of the most advanced home-based care workflow technology on the market, helping us solve key challenges and deliver tangible outcomes for providers across the country.”
