Homecare Homebase provides customers an on-ramp to the Carequality framework through Surescripts with HCHB Exchange

Dallas, TX – Homecare Homebase (HCHB), the nation’s leader in mobile home health and hospice software has announced the new HCHB Exchange product at the company’s annual Users Conference this week in Dallas.

Homecare Homebase’s partnership with Surescripts, a Carequality Implementer, includes access to clinical data of all other Carequality Implementers including participating CommonWell Health Alliance Service Adopters whose provider sites are live on the CommonWell-Carequality connection. The Carequality framework represents over 600,000 individual care providers, 40,000 clinics and 1,700 hospitals nationwide.

HCHB Exchange is part of the larger HCHB Connect Suite, a collection of tools to enable patient data sharing with health systems, ACOs, HIEs and the broader healthcare community. The rapidly growing post-acute segment now demands a higher level of data sharing between multiple sources – including clinicians, facilities, suppliers and all those who play a crucial role in the health and well-being of their patients. The HCHB Connect Suite enables, supports and streamlines patient referrals, clinician signatures, Clinical Information Exchange, Population Health Management and Messaging via real-time, two-way data shared by a patient’s entire circle of care.
