HCHB Customers Awards
Celebrating Care Quality, Community and Innovation
Homecare Homebase (HCHB) proudly supports its customers in the home-based care industry by celebrating those dedicated to quality care through the HCHB Customer Awards. Annually, HCHB recognizes outstanding agencies with two excellence awards and three achievement awards based on nominations and are presented during Users Conference.
Customer Excellence Awards
Quality Award: Given to Home Health agencies achieving a 5-star rating in the CMS Quality of Patient Care metrics. Hospice agencies are given this award when they achieve a 100 in the Hospice Quality Reporting Program (HQRP).
KPI Excellence Award: Honors Home Health agencies excelling in Acute Care Hospitalizations, which provides key insights into patient care and outcomes. Hospices agencies are recognized based on Hospice Visits in the Last Days of Life (HVLDL) which concentrates on visits provided in the last three vulnerable days of life for a patient.
Customer Achievement Awards
Industry Advocate Award: Highlights individual contributions in patient care, performance, innovation, or regulatory affairs.
Community Impact Award: Celebrates organizations making significant differences in their communities through extraordinary actions or programs.
Innovation & Achievement Award: Acknowledges organizations finding innovative ways to address challenges within the home care market.
These awards are based on nominations and reflect HCHB’s commitment to fostering excellence and innovation in home-based care.

Congratulations to the 2025 winners of The HCHB Customer Awards. The winning organizations were announced during the 2025 HCHB Users Conference in Dallas, Texas.
HCHB Customer Awards: Quality and KPI Excellence
- The 2025 Home Health Quality Excellence Award recognizes Home Health agencies achieving 5 stars in the CMS published Quality of Patient Care star rating.
- The Home Health KPI Excellence Award recognizes Home Health agencies performing in the top of their size cohort for Acute Care Hospitalizations. This metric has become well known in recent years due to new value based care models and proves to be a measure important in patient care and outcomes.
- The Hospice Quality Excellence Award recognizes Hospice agencies with a score of 100 in the Hospice Quality Reporting Program (HQRP).
- The Hospice KPI Excellence Award recognizes Hospice agencies performing in the top of their size cohort for care provided at the end of life. This award is based on the The Hospice Visits in the Last Days of Life (HVLDL) metric which concentrates on visits by at least one RN or Social Worker in the last three days of life. This metric measures care provided in the most vulnerable of times.

HCHB Customer Awards: Industry Impact
- Innovation and Achievement Award is a nomination-based award that gives Homecare Homebase the opportunity to highlight organizations that have found creative and innovative ways to address challenges within the ever-changing home care market.
- The Community Impact Award is a nomination-based award that will be given to an organization whose mission and efforts are positively moving the needle and making a significant difference in their community. Through extraordinary actions or programs, lives within the community are improved.
- The Industry Advocate Award is a nomination-based award that gives Homecare Homebase the opportunity to highlight the contributions of specific individuals for their excellence in patient care, performance, innovation, or regulatory affairs.
