What leaders have to say about Tomas:

Tomas is a very smart and passionate nurse who spends a lot of quality time taking care of his patients and their family, listening to them, looking for resources to help them and making the end of life an exceptional experience for them! He gets involved with family matters and goes above and beyond to help them with making proper decisions, solving family dynamics and getting essential paper works, including DNR/DNI forms done. He is curious about the care that his patients receive and shows enthusiasm to learn about their commodities, medication effects, and side effects. He is an amazing team player and always makes himself available to help. He is an amazing father for his 3 kids, a great husband, spends a lot of time in nature and it is a pleasure to work with him.

What does being a Gold Standard Award honoree mean to you?

It’s nice to be recognized for the hard work that our entire team does. Taking care of these patients is definitely a team effort and involves all of us. It is important to recognize the good work that we do and to take a moment to appreciate the change that we make in people’s lives.

What is a valuable lesson you’ve learned from working in your field?

Doing this work I have learned to stop and listen to the patients. To engage in active listening and truly take time to hear and be present. This has completely changed my interaction with patients and their families.

What guidance or words of wisdom would you share with those who want to follow a similar path to yours?

I would recommend that they take time to truly be present during the visit.

HCHB Responds to CMS’s 2027 Hospice Proposed Rule

HCHB’s 2027 Medicare Advantage comment letter supports CMS’s broader goals but argues that Medicare Advantage policies still create major barriers to timely home-based care through inconsistent prior authorization, weak continuity protections, and quality incentives that do not fully account for the needs of medically complex patients.

HCHB Personal Care is presented as an extension of the Homecare Homebase platform that helps agencies manage personal care alongside other home-based service lines through a single patient view and a more unified operational model.
