Ann Berthiaume deserves recognition of her exceptional dedication, compassion, and professionalism as a hospice medical social worker. Ann consistently goes above and beyond to support both patients and families during some of the most difficult moments of their lives. Her empathy, patience, and personalized care make a profound difference, offering not just professional guidance, but emotional comfort. In addition to her clinical skills, Ann displays a deep commitment to enhancing the quality of life for patients, addressing not only their physical needs but also their emotional, social, and spiritual well-being. She creates a safe, open environment for families to express their concerns and ensure every patient’s wishes are respected. Ann’s unwavering support has brought peace to countless families, and her positive impact is felt deeply by all she serves. Ann is very caring and provides exceptional expertise to Pathways’ patients. Ann enters very detailed notes in HCHB, describing many aspects of a patient’s mental, physical and spiritual state of mind. When reading the notes in HCHB, other members of the team are able to quickly grasp the needs of the patient and how to best support the patient in their own role. As a new employee, in a new role as Volunteer Coordinator, Ann’s thorough descriptions in the HCHB coordination notes has helped me identify patients that are appropriate for volunteer visitors and also help me pair patients with a suitable volunteer. Ann often lists patients interests snf hobbies, helping me pair volunteers with similar interests. Being able to access Ann’s notes in HCHB on patients’ needs has made it made my job a lot easier and more effective when pairing patients with volunteers. Ann is a very compassionate and wonderful social worker!! She provides emotional and social support to hospice patients and their families with empathy and wisdom. She is an incredible human being with a big heart for everyone!! We are so fortunate to have Ann on our Hospice Team here at Pathways Healthcare LLC, Londonderry, NH Branch Office.
What does being a Gold Standard Award honoree mean to you?
It is a privilege to work alongside an amazing team of Healthcare professionals at Pathways Healthcare. Our hospice team serves patients and families in greater southern New Hampshire with compassion and dignity through the end of life journey. I strive to uphold the organizations values of teamwork, respect, understanding, service and transformative care.
What is a valuable lesson you’ve learned from working in your field?
Trust your training, skills and experience. Trust in your team members expertise and always lead with your heart with great compassion. When you are part of a team no challenge is too great.
What guidance or words of wisdom would you share with those who want to follow a similar path to yours?
It truly is a privilege to share in the experience of hospice patients and their families. Yes, we share tears and hold space for people to express their fears and worries. We are teachers, friends and experts all rolled in one. We also laugh, tell stories, and bring joy, peace and dignity as we help people finish out their life’s work.
