Erica is an exceptional Case Manager. She maintains her caseload well, completes her documentation same day, and ensures continuity of care and proper care coordination. on top of that, Erica is a PointCare star and serves as a resource for her peers on navigating the system. She even conducted training courses when we rolled out supply requisitioning. Erica truly has embraced HCHB and has fully integrated it into her patient care practice.
What is the most fulfilling part of your role?
I have been a nurse since 2011 and have been in the Home Health field since 2016 and utilizing Homecare Homebase since then. What a love about Home Health is the actual one on one time we have with patients to provide education and care without the constant interruptions we have in the hospital setting. Patients are more comfortable at home and tend to be more receptive to the education and care being provided to them which leads to better outcomes with their health.
