Homecare HomebaseSM (HCHB) announced the winners of its 2024 Client Success Awards honoring the outstanding contributions to home-based care made by HCHB customers. Four awards celebrating excellence in quality metrics and key performance indicators (KPIs) were presented.
The Home Health Quality Excellence Award, given for achieving a 5-star rating in CMS Quality of Patient Care, went to Americare CSS, GrandCare Health Services, Healing Hands Healthcare, and High Plains Senior Care, part of Frontpoint Health. The Home Health KPI Excellence Award, recognizing top performance in Acute Care Hospitalizations, was awarded to Community Nursing Services, One Home Health, and VNS Health.
The Hospice Quality Excellence Award applauded perfect scores in the Hospice Quality Reporting Program (HQRP), with winners including Baystate Home Health & Hospice, Careline Health Group, Caris Healthcare, GENESIS, Gentiva Health Services, Knute Nelson, Southeast Health and VNA Care. The Hospice KPI Excellence Award, measuring end-of-life care quality through The Hospice Visits in the Last Days of Life (HVLDL) metric, was given to Gentiva Health Services, Moments Hospice, and Tabitha.
Three awards were also presented based on nomination Homecare Homebase received for outstanding service in the following categories.
The Innovation and Achievement Award highlighted organizations that have found creative solutions to the challenges faced by the home-based care industry. HarmonyCares was noted for efficiency improvements and reduced readmissions, while Visiting Nursing Association of WNY achieved ISO 9001 Certification for quality management practices.
The Community Impact Award recognized St. Croix Hospice and Stillwater Hospice for their extensive community programs. The Industry Advocate Award honored Rachel Witt and Ashley Jones for their leadership and advocacy in home-based care.
